[updated:LAST EDITED ON Apr-27-05 AT 07:44 PM (CST)]Cut out the damaged areas and weld in new sheet metal.
Search the online store for patch panels if you can't find what you need there you will have to fabricate the pieces yourself.

If it's the hinge pillar that is rusted out. STOP NOW and check the roof around the drip rails. Door pillars usually rot from the inside out caused by water leaking through an unsealed drip rail. The water leaks in, runs down in between the inner and outter sheet metal and rusts out the pillars. It also causes the floors to rot where the kick panel meets it. Because the hinge pillar is usually thicker, and takes longer to rot, Chances are good that there is MORE rot than just the hinge pillar. Reassess the cab and determin if it is worth saving before you start sinking money into fabricating hinge pillars.

Fixing the hinge pillar will require that you find a doner cab and graft a good pillar to your cab.
