A clutch plate won't work if it's in backward so something is moving the plate and springs towards the bolt heads. Could the plate be wrong and have springs that are too large in diameter? I guess it's possible for a flywheel to be resurfaced enough times to move the plate closer but I've seen one that the entire lining has come off the plate and don't recall them hitting the bolt heads (although they didn't go far without the lining on). When you get a new plate put some Play Dough on the head of each bolt, bolt everything up, then take back apart and see how much clearence you have by measuring the thinnest parts of the now mashed to fit Play Dough.
